Participants

N = 103 health providers (77 public facility providers, 26 private facility providers) across 93 primary healthcare facilities in Nairobi, Kenya.

Study Design

OLS regression interacting accountability treatment assignments (Peer Disclosure and Monetary Penalty) with individual baseline reciprocity scores (returned-received ratio measured during a preceding standard Trust Game).

Results

The interaction between accountability treatments and baseline reciprocity was consistently negative (e.g., Monetary Penalty x Baseline Reciprocity coefficient = -0.269, p = 0.022), demonstrating that accountability sanctions selectively increase prosocial performance among unmotivated workers who have higher baseline absenteeism.

Takeaway

Target peer disclosure and reputational oversight mechanisms specifically at chronically underperforming public health facilities rather than imposing universal oversight across all staff.
